Please, be sure to read and understand everything carefully so there is no confusion about what to expect and what is expected of you during your stay.
If you are joining our sober living please fill out all 3 forms upon move in, Sober Living Intake, and Peer Coaching Intake, and Intake Contact Info form.
If you are only a Peer Coaching client please fill out only the Peer Coaching Intake and Contact Info Form below.
Also all Hazelbrook Residents have unlimited access membership to ParadigmONE Sober Community Center which includes boxing program, fitness and weightlifting, art program and more!!! Click the red text above!
If you have any questions feel free to ask the Senior Resident at your house.

Drug or alcohol addiction usually has been going on for many years and it takes a little while for your mind to re-adapt to a normal life.
We urge residents to stay with us for at least days 90 but they are welcome to stay as long as they want. All you need to do is sign up using the buttons below. Thank you and God Bless!
